Register as a New Patient

 Area

Our surgeries are based in Dorchester, on the edge of the new Poundbury Village, and Portesham. We cover a semi-rural and rural area stretching from Burton Bradstock and Shipton Gorge to the west, Godmanstone and Piddlehinton to the north, to Puddletown and Crossways to the east.

Registration process

Anyone can register with us provided they live within our catchment area. To register complete the forms below and click on the "Submit" button to mail your forms to us. When you visit your chosen Surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Your Doctor

All patients are allocated a Named GP, and for continuity of care we encourage patients to see their Named GP whenever possible. You can express a preference for a GP and we will try to comply with your request where possible.

Proof of Identity

Acceptable proofs of identity are photo ID such as a photo driver’s licence, passport or fire arms certificate. If you do not have any photo ID then please bring your birth certificate, or a marriage, civil partnership, or divorce certificate. You will also need proof of address, such as a Council Tax Bill, utility bills, mortgage statement, bank/building society statement, benefits statement or work permit/visa. Proofs of address need to have been issued in the last 3 months.

Temporary Registrations

If you are currently staying within our practice boundary and need urgent medical care, please contact your registered GP in the first instance to see whether they can continue to support you remotely e.g. by sending a prescription for your medication to one of the local pharmacies.

If your registered GP is unable to assist and your illness requires urgent treatment then you can contact the Surgery and ask to register as a temporary resident. We will ask you to complete a temporary registration form.

You can register as a temporary resident with a GP in England if you are in their area for longer than 24 hours but less than three months.

Please note:

  • Practices do not have to accept you as a temporary patient though they have an obligation to offer emergency care.
  • You cannot register as a temporary patient at a practice in the town or area where you are already registered.

What does temporary registration mean?

You will still remain registered with your permanent GP, but you can see a temporary GP for up to three months. After three months your temporary registration will expire and you will be asked to  permanently register with the practice.

As your temporary practice, the GP will pass details of any treatment you have to your permanent GP, who will add the information to your medical records.

What information do I need to register?

In order to be able to treat you appropriately as a Temporary Resident the following information is needed:

  • details of any medical conditions you have
  • details of medical conditions you’ve had in the past
  • the name of any medicines you’re currently taking
  • details of anything you’re allergic to
  • contact details for your permanent GP
